1. I understand that My Med-Aura.com is intended as a secure, online source of confidential medical information. If I share my username and password with another individual, that person may be able to view my health information or the information of anyone else that I am a proxy for within the patient portal.
2. I agree that it is my responsibility to select a confidential password, to maintain my password securely, and to change my password if I believe it may have been compromised.
3. I understand some of my medical information may not be available in my Med-Aura/My Chart. I also understand I can file a request for electronic or paper medical records online or through My Med-Aura, or I can ask for a record request form from staff.
4. I understand that my activities within My Med-Aura/MyChart may be tracked by computer audit and that entries such as messages or demographic changes I make may become part of the patient’s medical record.
5. I understand that access to My Med-Aura/MyChart is provided by Med-Aura and Med-Aura Physicians as a convenience to its patients, and that Med-Aura and Med-Aura Physicians have the right to deactivate access to the portal at any time for any reason. I understand that use of Med-Aura/MyChart is voluntary, and I am not required to use the portal or grant an individual proxy access to my account.
6. I acknowledge that I have read and understand this Med-Aura/MyChart Sign-Up Form, and I agree to its terms.
